REQUIREMENTS FOR ATTENDING - Students MUST be:
1) Age 21, or younger
2) Have a valid Learner’s Permit or Driver’s License at the time of the class
REQUIRED FORMS – MUST have for all students under age 18:
Parental consent form. (1 page)
AND
LVMS Minor Waiver (2 pages)
We need the original forms. Mail or fax the consent form to us.
If you fax it, please also BRING the original to the class with you.
